HERON McMillan Marsh improves IDX/O2 encounter mapping and introduces Alerts

Looking for medications prescribed at an outpatient surgery visit? Want to limit patients with an outpatient surgery to those with a specific diagnosis at the time of surgery? Now you can. HERON McMillan Marsh maps IDX and O2 encounters resulting in the ability to link O2 concepts, such as diagnosis and medications, with IDX concepts, such as outpatient procedures, with the same financial encounter temporal constraint.

New data types

  • Best practice and medication alerts are accessible in the Alert folder. This folder is in draft mode, and your feedback is welcome.
  • Emergency Episode is a new field available in the Visit Details/Encounter Type folder. This field allows you to search for patients seen in the emergency department regardless of where their encounters originated or terminated. The Emergency field, also in the Encounter Type folder, represents patients who left the Emergency Department without being admitted to the hospital.

HERON Webster release finely tunes data fulfillment and fixes search timeout issue

HERON searches would stop running and appear as processing indefinitely once timed out (~180 seconds). HERON Webster release fixes the issue. Once a search times out, it will continue to search in the background. Check the query in your previous queries section to see if the search completes.

This release includes back-end enhancements that will allow for more precise data delivery when requesting data. Searches limited by dates will now provide only data within those date ranges. Improvements to identified data requests are also included. Learn more about requesting data.

Outstanding Problems/Defects/Issues

HERON turns 45 with the Kanopolis Release and includes temporal query reuse fix and new concept, IDX Place of Service

The HERON Kanopolis release marks the 45th update to HERON. Over the past 46 months, HERON has grown in terms of data and functionality. Temporal Queries, ability to filter searches through modifiers, and improvements to the Timeline tool are just a few examples of new functionality. HERON continues to grow through the PCORI Greater Plains Collaborative (GPC) work toward standardization of concept hierarchies, and the amount of medical data within HERON has grown from approximately 300 Million in the early releases to over 1.68 Billion medical observations in this release.

HERON Kanopolis new features (concepts):

  • IDX Place of Service, new concept that identifies the physical setting for outpatient/clinic-based services (office, clinic, inpatient hospital, outpatient hospital, emergency room hospital and others).

HERON Kanopolis fixes:

  • Inability to reuse temporal queries
  • Same financial encounter mapping between UHC and O2 data

HERON Kanopolis known issues:

  • 8300 UHC to O2 encounter mappings are lost. The HERON team is troubleshooting this issue.
  • Flowsheets observations decreased due to data clean-up in O2.

The Kanopolis release includes data through November 2014.

HERON Verdigris: Search laboratory tests by reference ranges

Don't know the exact reference ranges for a certain lab? No problem since you can search a subset of HERON labs by High, Low, or Normal ranges. To search lab ranges:

  • Drag a lab into the search group. The value window will display if a value exists for this lab
  • Select By Flag on the value window
  • Select Normal, High, or Low
  • Click OK and continue building your search.

Note that the value window will not appear for some labs. For example, the influenza laboratory test returns either positive or negative. As a result, you cannot search by value or range.

Temporal searching available in HERON Delaware release

Please note: Clear your browser cache if you receive an error when starting the HERON query tool.

Instructions to clear cache for Firefox and Chrome.

HERON Delaware upgrade to i2b2 v1.7.03 introduces the Temporal Query Tool which allows you to narrow your search by a series of events. For example:

  • Define a population
    • create a search for males with hypertension and acute myocardial infarction
  • Define events
    • Event 1 is hypertension
    • Event 2 is acute myocardial infarction
  • Define temporal relationships of the events
    • Event 1 happens before event 2 by greater than 1 day
  • Submit the query.
    • Results include the number of male patients who have had at least 1 acute myocardial infarction diagnosis after a hypertension diagnosis.

HERON issues to note due to recent data clean-up by the O2 team:

  • Deceased per Social Security Master Death File decreased due to the removal of certain Social Security Numbers in the EMR. See #2911 for more details.
  • Nursing data decreased. See #2955 for more details.

HERON Wakarusa include UHC fix and Fishing Trip update

A University HealthSystem Consortium (UHC) format change for Sept 2013 to June 2014 data resulted in the exclusion of a small portion of UHC data from HERON. This issue is fixed with the HERON Wakarusa release.

The 2nd Annual HERON Fishing trip took place on July 28, 2014. Agenda and recorded sessions are available. A demo of i2b2 version 1.7 gave the attendees a glimpse of upcoming temporal searching capabilities. HERON is built upon i2b2. Look for this new functionality when HERON moves to i2b2 version 1.7 in the future.

To learn more about HERON or REDCap, join the Frontiers Lunch and Learn which meets the 1st and 3rd Tuesdays of each month from 12:00 - 1:30 PM in room 3001D Student Center. Register in advance if possible but drop-ins are welcome.

HERON Hackberry release includes emergency, inpatient and outpatient encounter data

Encounter types, such as inpatient, outpatient and emergency, are located within the Visit Details folder. Including or excluding specific encounter types increases your search precision, especially for common items such as an A1C test, by narrowing your search to a specific type of visit. Procedure tool tips are once again visible in this release.

The HERON Marmaton includes two new data types.

  • Cardiology lab data - search for lab orders such as Ejection Fraction, Vascular Ultrasounds, Echocardiogram, and more.
  • BMI Percentile - search BMI by CDC defined ranges for the KUMC pediatric population.

HERON Republican release allows you to limit laboratory searches to a value for tests with a value assigned. If a lab test does not include a value (e.g., testing positive or negative for influenza) the option to limit by value will not appear. This release also introduces outpatient billing service lines and microbiology positive results.

Exciting changes are coming for the Frontiers Clinical and Translational Informatics Clinic. This walk-in training clinic will be held over the lunch hour on the first and third Tuesday monthly starting May 6th. Searching clinical services in HERON is no longer limited to inpatient with the introduction of ambulatory clinical services in the Beaver release. To search on a specific outpatient clinical service, go to Visit Details -> Clinical Services -> Outpatient Visits.

KUMC receives PCORI award to lead Greater Plains Collaborative

The Greater Plains Collaborative award from the Patient-Centered Outcomes Research Institute (PCORI) provides $7 million for a project that will establish a new network of nine medical centers in seven states committed to building a data set from electronic medical records that will be used to contribute to new research in the fields of breast cancer, obesity and amyotrophic lateral sclerosis (also known as ALS, or Lou Gehrig's disease).

The principal investigator of the project is Russ Waitman, director of medical informatics at KU Medical Center. The HERON technology developed by the medical informatics team plays a prominent role.

HERON Lovewell improves navigation and fixes Frontiers patient count issue

HERON Lovewell release facilitates navigation by introducing changes to the abridged Cancer Cases Identifier structure. Date ranges are added to AJCC concepts, and folders are rearranged into an intuitive order. The issue of inaccurate Frontiers Research Participant Registry patient counts ((see #2451)) is resolved. Searches will return the correct patient count when searching with the Frontiers Research Participant Registry concept.

HERON Toronto update powered by newer, faster hardware and load process

With HERON Toronto we are on newer and faster hardware. Users will likely notice faster query response times. We have also streamlined our monthly process of loading data from various sources into HERON so that we can deliver the data to users faster.

With this release, data in REDCap SQL fields will also be loaded into HERON. The browsing experience for these fields will be indistinguishable from multiple choice fields. To add your REDCap project to HERON, register your project after going over the directions for readying your project.

Please note: Searches involving Frontiers Research Participant Registry patients will return an incorrect number. See #2451 for more information.

HERON Milford simplifies the Cancer Cases folder

A new folder, Cancer Cases (Abridged), debuts with the Milford release. This folder contains the frequently searched concepts from the Cancer Cases folder. Advanced searchers can still search the entire tumor registry in the unabridged folder.

The Milford release marks the 8th REDCap database integrated into HERON. When you add a REDCap patient registry to HERON, you enhance your HERON searching capabilities while at the same time enhancing your REDCap registry.

Integration of REDCap project data into HERON is driven by our desire to empower the researcher to:

  • look at a richer data picture of their project population.
  • look for disparities in data in the EMR versus the data in REDCap projects.

Since July 2012, REDCap projects that model breast cancer, Alzheimer's, Sepsis management registries among others have been integrated into HERON.

How to prepare your REDCap project to be loaded into HERON?

Integration of REDCap project data into HERON is driven by our desire to empower the researcher to:

  • look at a richer data picture of their project population.
  • look for disparities in data in the EMR versus the data in REDCap projects.

Since July 2012, REDCap projects that model breast cancer, Alzheimer's, Sepsis management registries among others have been integrated into HERON. In this post, we briefly summarize how you can get your REDCap project ready for integration into HERON.

Make sure you document the MRN for patients in your project.
REDCap data related to a patient is linked to the rest of their EMR data using their Medical Record Number (MRN). Please make sure you record the MRN accurately for the patients with all the leading zeros. Patients missing valid MRN will not be loaded into HERON.

Verify that no two records in your REDCap project have the same MRN.
If two records(i.e. patients) have the same MRN, then your REDCap project should be modeled as a longitudinal project. If you have questions regarding longitudinal projects, please watch the relevant video on https://redcap.kumc.edu/index.php?action=training. If you need further help changing your project to be longitudinal, contact us.

Mark the fields containing PHI as "Identifiers"
If you are recording PHI in a field in your REDCap project, please make sure you have marked that field as an identifier. You can do that from online designer. Once a field has been marked as an identifier the data in it will not be loaded into HERON so that we can comply with our "de-identified data only" policy.

Check the REDCap field types in your project
Right now we only pull the following REDCap field types into HERON:

  • text fields with integer, float or date validation. We ignore text fields without any validation.
  • Radio, checkbox, select(dropdown), yesno fields

Make sure the data you want to look at in HERON is in those fields. If not please contact us and we can try to expand this list. We do not load data from unvalidated text fields due to the de-identification challenges.

Verify the user rights to your project
Currently access to your REDCap data in HERON is limited to only users who have permissions to it in REDCap.

Project Status: Production
Finally, make sure your project has been moved to Production. This is a good practice for REDCap projects as it helps protect the project structure and data against inadvertent changes.

Our releases are around the middle of a month
For your project to be part of the next monthly release, the project has to be ready by the first week of a month. We will first stage the data and will contact you if you we have any obvious issues that we need your help in resolving.

HERON Solomon marks the 32nd consecutive release

Over the past 32 releases, HERON grew in data, functionality, and use. New data types include REDCap projects, Biospecimen Repository, Tumor Registry information, and much more. In response to this increase, the Medical Informatics team lead the HERON Fishing Trip, an extended training workshop in August 2013. This two day event consisted of hands-on and classroom trainings with attendance both in person and online.

Since that workshop, monthly searches have surpassed 1000, Frontiers Research Participant Registry requests increased, and clinics are steadily attended. Researchers, study coordinators, students, and administrators from multiple areas of KUMC and KU Hospital search HERON. Thirty-two releases after the initial launch and HERON continues to grow.

The Solomon release includes clinical data through September 2013.

HERON Chikaskia release introduces HERON DUA to REDCap & KU ADC data

When you request data from HERON you can receive the data via a REDCap project. The data will be pivoted into a user-friendly format and available in REDCap CRFs. The data will reside on REDCap servers cutting down the use of email in sharing data.

Data from University of Kansas Alzheimer's Disease Center(KU ADC) REDCap project is now part of HERON. This project has a richly characterized cohort of over 300 Alzheimer's patients. You can access this data only if you have the permissions to the related REDCap project. Please contact Dr.Jeff Burns (jburns2@…) if you would like to look at these patients in HERON.

HERON is now on i2b2 v1.6.09. Queries including concepts with symbols like < are now working.

General fixes in this release include:

  • HERON databases are now running on a newer version of Oracle.
  • UHC data is now under the folder University HealthSystem Consortium
  • HERON help button is functional again.

HERON Crooked Creek release introduces the HERON performance page

The performance page displays the status of searches, those complete versus incomplete, and performance times within HERON. A link to this page is located below the start query tool link.

General fixes in this release include:

  • The disappearing shared folder issue for those users with REDCap project access within HERON is resolved by allowing the user to choose between a REDCap HERON and a general HERON instance (e.g., in this month the Crooked Creek instance). The shared folder will not appear in the REDCap instance, but users can switch back and forth between the 2 HERON instances access the shared folder.
  • The Survival Analysis tools work in Internet Explorer. Previously, patient sets could not be dropped into this tool in the IE Browser.

Note: The latest University HealthSystems? Consortium (UHC) data remains at December 2012. The inability to pull new data into HERON is due to a table change within the database which the team is working to resolve.

HERON Medicine Lodge introduces Microbiology Lab results

Microbiology laboratory results are searchable by source, organism, antibiotic, or sensitivity levels. Patient statistics are corrected to diagnosis display the correct number, and patients will no longer appear in ICU Length of Stay results unless they have an ICU stay of 1 day or greater.

HERON Smoky Hill release enhances the timeline tool and limits REDCap access

Name and value (if applicable) are now available in the Timeline tool when hovering over an event. This enhancement expands the data analysis capabilities within HERON. For example, if you are interested in when antineoplastics were received in a patient set, you can add the antineoplastics folder to the timeline tool. Upon hovering over each event tic name of the antineoplastic and value if provided is displayed. Previously, users could easily see when the antineoplastic was administered in relation to a diagnosis, but the addition of these labels and values allows the users to see if a patient received more than one antineoplastic type and when that change occurred.

Access to REDCap projects in HERON is now limited to only those users defined in the REDCap project. If a user is not a member of the project, the project will not be visible within the HERON navigation tree to that user. It will be visible to project members who can add the project to queries within HERON.

HERON Cow Creek release includes order sets and analysis tool enhancements

Previously, the Multi-Cohort Survival Analysis tool had the option to limit by 5 years, 10 years or All. This release expands that option to weeks, days, or hours in addition to years. The Timeline analysis tool increases granularity to display by the hour as opposed to day as in previous releases.

The Cow Creek release introduces order sets as a new searchable data type. This release also kicks off the ability to limit MAR by dose.

The Neosho update replaces numerical codes with text labels for tumor registry primary site and histology. Support for user-selected outcomes in the multi-cohort analysis tool has been fixed (though initialization issues remain). New UHC data includes low socioeconomic status and admission source.

Previously, limiting searches to the same financial encounter meant increased query times. This release fixes the issue resulting in quicker queries.

HERON Council Grove update includes histories, encounter vitals and expanded smart notes

The Council Grove release introduces family and social histories within the HERON tool. Family history can be searched by all or by a particular family member type (e.g., Maternal Grandmother). Additionally, a new modifier allows searching by medical history within the clinical diagnosis folders. Other data changes within HERON include encounter vitals, which are searchable under the Visit Detail folder, and smart data notes, which are expanded to include ambulatory in addition to the previously released inpatient notes.

HERON Elk City updates to i2b2 v1.6.07 and includes Ambulatory Nursing Documentation and Orders

The Elk City HERON release includes an upgrade to the i2b2 software version 1.6.07. Width of the Query Tool and Navigate Terms workspaces can be resized by dragging the vertical bar between the areas. Query within query fixed to provide correct patient numbers. We have also incorporated nursing/multidisciplinary flowsheet data from the ambulatory clinics (an additional 20 million facts) as well as procedure orders (from 54 million to 110 million).  For the flowsheet data, check out the "UKP" folder and the "KU AMB" and "KU GEN CARD" subfolders under "KU" in the ontology for many of the new types of data now exposed by HERON for clinic visits.  This release also includes many fixes behind the scenes not visible through the interface.

Note that this month, the Smart Data elements contained within physician notes were not loaded due to a data inconsistency detected by our loading process. We are investigating the root cause but do not want this fairly new data source to delay the release of the database for this month. See ticket 1516 for details.

HERON Melvern update allows search by category and enhanced medication dose modifiers

Melvern update includes a new way to narrow a term search. Within the "Find Terms" tab, you can limit your "Search by Names" to a specific category, such as flowsheets or diagnosis. Additionally, you may notice the top-level folder (entitled HERON) is removed to allow less clicks to browse for terms.

Additional dose types are added to medication modifiers.

HERON Walnut update introduces O2 smart phrases, medication by dose and age at visit searching

In O2, smart phrases, commonly used words or phrases, are used to expedite patient documentation. Find these phrases under Reports->Visit Notes->Note Concepts.

Search medications by dose for inpatient medication orders. When constructing a search, drag "Cumulative Daily Dose of Single Inpatient Order" and you will be prompted to enter desired dose criteria.

Age at visit is searchable for patient visits. Please note this is based on de-identified dates. See HERON training for more information on how dates are shifted.

HERON Waconda update introduces new searching modifiers for Medications and Diagnosis

Searching with the Medical Administration Record (MAR) concepts limit your search to only medication administered in a specific way. For example, you can search for bolus in a bag or exclude medications denied. Diagnosis modifiers allow you to specify whether to search electronic medical record(O2), billing (IDX), or quality measures (UHC) diagnosis data. Both types of modifiers offer you the flexibility to narrow your search criteria when desired thus increasing the precision of search results.

UHC Core measures, Standard Medication Terms, and REDCap integration added to HERON in Cimarron release

Cimarron release includes a new structure for browsing data (medications), addition of a new data repository (REDCap), and new data concepts (UHC) plus bug fixes.

  • We can now import data from selected REDCap projects into HERON. This integration effort was piloted with a Cancer Registry.
  • Medication navigation improved due to organization by RxNorm based on GCN or NDC codes instead of clinical drug level, such as pill size. 89% of medication is mapped to RxNorm and we continue to work on the remaining 11%.
  • Visit details for inpatient visits provides insight into what services were assigned to a patient during their hospital stay.
  • Three important new types of UHC data available
    • Physician role and specialty are available to search in the UHC Visit Details folder.
    • Core Measures are standardized, evidenced based measures used by UHC to evaluate hospitals. These measures were designed by the Joint Commission to permit more rigorous comparisons of hospitals.
    • Numerator or denominator only case flag is added to AHRQ. See more information on AHRQ.

The Hillsdale release makes usage statistics available to HERON users. By clicking on 'Usage Report' you can view monthly query counts and queries by users over time. Average query time improved and we will continue our work on optimization. Marital status now displays correct patient count.

HERON: Marion release increases UHC and medication data

The Marion release adds ICD-9 codes for both procedures and diagnosis, in addition to increasing year range to include 2009-2012 for UHC data. Inpatient and outpatient medication orders are now available. Marion release also includes two bug fixes: searching with the exclude limit now returns the correct results and password issues no longer make the system hang.

Since February 2012, search performance is slower than expected in HERON.  We are working to resolve this issue (#1069).

Health of the Public poster session shows time-saving benefits of Informatics Tools

The most recent Health of the Public (HOP) student poster session in April provided another glimpse into student research at KUMC. I am continually impressed by the hard work of these students to create a project, collect data, analyze the data, and present within four weeks. The best part of the HOP poster sessions, for me, is speaking with the students to learn how they employ our informatics tools, what worked or didn’t, and seeing the impact of our tools on student research in general. In this session, the majority of the posters mentioned REDCap, and a few highlighted HERON. Many students praised our tools and in particular, the time saved in collecting and analyzing the data. Each poster session presents new questions regarding public health, new ways in which the data is collected and analyzed, and an increased role for informatics tools. I am looking forward to seeing what the next poster session in October holds.

HERON: Big Hill release adds home medications, other medication orders, and expands UHC data

The Big Hill release adds support for home medications and expands UHC coverage. Previously, we only had medications that are dispensed by the hospital pharmacy; now, you'll be able to see

  • medications that the patient reports taking at home (so called "historical medications"), as well as
  • all other orders, which includes prescriptions, inpatient medication orders, and discharge medication orders.
  • data for an additional 100K patients

The UHC data, although primarily administrative, provides a new view on a patient's interaction at the hospital. New UHC concepts include:

  • ICU length of stay: Search on time spent in ICU.
  • Admission and Discharge status concepts: Search where patients came from prior to admission or went to upon discharge.
  • Readmission concept: Helps you search for patients readmitted after discharge to their home, allowing specification to a desired number of days.
  • Clinical Classification Software (CCS) ICD-9: These codes provided by AHRQ collapse ICD-9 diagnosis and procedure codes into a smaller number of categories useful in analyzing data. See AHRQ web site.
  • All Patient Refined Diagnosis Related Groups (APR DRGs): DRG codes expanded to include 4 subclasses in severity of illness and mortality subgroups for each code. See web site.
  • Major Diagnostic Categories (MDCs): Search these diagnosis categories created by combining ICD-9 diagnosis codes into 25 MDCs. These codes, which are used primarily for administrative and billing purposes, provide another view on patient data.
  • Comorbidity: Search 29 comorbidity categories.

Currently the UHC data is limited to a 1-year time period (Nov. 2010-Nov. 2011). Look for additional years in future releases. This data is limited to hospital encounters and lacks clinic data.

HERON El Dorado Release incorporates searching by MSDRG and LOS

This release includes hospital quality measures (UHC) data integration which enhances search capabilities by introducing new concepts, such as length of stay and MSDRG. 109K UHC observations from the November 2010 - November 2011 are included in this release. This is our first step in making UHC data available. Look forward to additional UHC data in future releases.

Adding SEER Site Recode to HERON Tumor Registry integration

Our HERON tuttlecreek release a couple months ago included initial integration of data on ~60,000 cancer cases from the KUMC tumor registry. We organized the NAACCR terms based on work by colleagues at the Kimmel Cancer Center in Philadelphia and Group Health Cooperative in Seattle:

But if you want to find, for example, brain cancer cases, due to an outstanding issue (#733), you have to be an expert in codes for primary site, histology, etc.:

For our next release, based on work with John Keighley, we're providing query by SEER Site Recode, a state of the art method for combining primary site and histology:

Under the hood: Using python to convert the rules table to SQL

The SEER Site Recode ICD-O-3 (1/27/2003) Definition, lays out the rules in a fairly convenient HTML table:

Converting that table to code manually might have been straightforward, but it would have been repetitive and error-prone; so like so many Geeks and repetitive tasks, I wrote a script to automate it.

source:tumor_reg/seer_recode.py weighs in at about 200 lines, including whitespace and a handful of test cases. It reads the HTML page (well, I feed it through tidy first to clean up some table markup) and produces

  1. A term hierarchy in CSV format (source:heron_load/curated_data/seer_recode_terms.csv)
  2. Rules to recode our our ~60K cancer cases as a SQL case statement (source:heron_load/seer_recode.sql).

The resulting SQL weighs in at about 500 lines. Handling all the different kinds of rules in the table was fun; a lot more fun than writing this sort of SQL by hand:

HERON Pawnee release paves the way to a richer data repository

Our Pawnee release is the first to use i2b2 1.6. This allows us to take advantage of i2b2 1.6 features in future releases:

  • medication routes, frequency

Note: our statistics regarding total number of flowsheet observations is reduced significantly from 470 million to 328 million. We are investigating the cause as the we think the data is intact (see ticket #789). Please give us feedback if you notice discrepancies with any prior queries where the counts are down significantly relative to last month's build.

Notice in the right hand side of the user interface that i2b2 1.6 allows you constrain observations to the same encounter. For example, you might require that patients have drug exposure (furosemide) and laboratory monitoring (serum potassium) during the same encounter while another observation like "diagnosis of diabetes" can be treated independently of encounter. Right now, we are not sure we have this working (see defect #790) but we are looking into it.

HERON Tuttlecreek release brings initial Tumor Registry integration

This month's HERON release integrates data from the KUH Tumor Registry, with 65,000 cases dating back to the 1950s(#547). We have also added support for finding patients in KCK county school districts (#531).

AMIA 2011: Nursing Flowsheets data and the wild west of terminology

Like a number of other CTSA sites, we're Using I2B2 in our HERON research data respository. The original domain of I2B2 was genome/phenome integration and personalized medicine. Genomic stuff is on our long-term radar, but one of our earliest wins has been mining the vast amount of data (~400M observations as of our latest release) recorded by nurses and other practitioners in flowsheets in our EMR.

Height, weight, and BMI are quite common inclusion/exclusion criteria for clinical trials, and those aren't available in the term hierarchy that comes with I2B2 out of the box.

It's been a big challenge because unlike diagnoses with ICD9 codes and procedures with CPT codes, the MedicalTerminologyMarketplace has no widespread norms for flowsheets.

We're presenting some results in Washington D.C. on Wednesday at AMIA 2011:

Social Security Death Index integration expands vital statistics available in HERON

The Department of Biostatistics is excited to provide for you linkage between our clinical and administrative records and the Social Secuity Death Master File released by the National Institute of Standards and Technology.

Previously, we only had record that a patient was deceased based upon follow up at KUH/UKP or if they died while cared for at KUH/UKP (23,850 patients indicated as "Deceased" within the "Demographics" ontology). Now we have an additional indicator that the patient has died based upon records reported to the federal government (177,706 patients indicated as "Deceased per SSA" within the "Demographics" ontology).

Note, we currently are matching patients based upon an exact match of their social security number AND their date of birth.

We think this will be a powerful addition which will allow preliminary hypotheses generation regarding mortality rates between different patient cohorts. In the future we might hope to provide analysis plugins that calculate routine survival analysis.

For a neurlogical example: HERON has 1356 patients who've ever been diagnosed with Amyotrophic lateral sclerosis (ALS aka Lou Gehrig's disease) at KUMC since 2000. Of those, the hospital knew 206 had died. Now that we can also check the social security administration, we know that 821 have died.

Using i2b2 Timeline and other analysis plug-ins in HERON

We recently worked out the technical and regulatory issues to take you beyond counting to detailed analysis of the data in HERON (#314).

To access these capabilities, choose to check the box to also create a "patient set" when you run your query. Then, use the analysis tools tab at the top which takes you to a series of i2b2 plugins. We have found that the "Demographics (1 Patient Set)", "Demographics (2 Patient Sets)", and "Timeline" plugins to be very illuminating.

Note that because we shift dates back in time (0 to 365 days) for de-identifcation, some of these results in HERON are noted as resulted in 2003. First Frontiers Clinical/Translational Informatics Clinic

We'd like to thank everyone who came to the first Frontiers Clinical/Translational Informatics Clinic yesterday.

The clinic is scheduled for 4pm every other Tuesday in Library Room 1040. As the announcement said:

The Frontiers Clinical/Translational Informatics Clinic exists for investigators needing assistance using informatics tools for data exploration, cohort discovery, and data collection. The clinic is a brainstorming session and an education forum for investigators and informatics to share knowledge, provide feedback, and optimize the use of systems such as: - REDCap for data collection - the HERON clinical data repository and its i2b2 application - and other systems developed as part of Frontiers: Kansas City's Clinical and Translational Science Award.

It was interesting to chat with everybody, from investigators with proposals in development to research nurses, study coordinators, etc. We introduced our systems and talked about how they can be used to support research.

CRIS? is KUMC's custom installation of Velos eResearch, the highly-regarded clinical research information system chosen by 20 medical institutions with top 25 ratings.

For those doing a clinical trial, specially for cancer or an investigational drug where adverse event reporting is requred, CRIS is the way to go:

REDCap allows you to build and manage online surveys and databases quickly and securely. It's used by thousands of projects at hundreds of institutions.

If you are building a patient registry or a lightweight trial at KUMC, try REDCap:

HERON allows you to explore de-identified data from Epic/O2 (the hospital electronic medical records) and IDX (the clinical billing system). We talked about capabilities and access rules for HERON, using some HERONTrainingMaterials? for reference.

Access to de-identified data is non-human subjects research and does not require approval by the HSC/IRB. For qualified faculty who want view-only access to do patient count queries, executing a system access agreement is the only requirement.

A Data Request Oversight Committee (DROC) oversees requests to:

  • sponsor collaborators and research team members
  • extract de-identified data sets

For access to identified data, you will need an IRB approved protocol.

We look forward to meeting with our user community again in two weeks.

Approaches to data integration for health care research: i2b2, SHRINE, SPARQL and OWL

A couple of us are off to Boston this week for the i2b2 Academic Users' Group First Annual Conference to present a poster and soak up all kinds of good stuff.

i2b2 is the basis of HERON, our health care research data repository, which stores clinical observations into a fairly traditional datamart. I've done database application development of various kinds for decades, but the scale and operational challenges are new to me. I'm particularly happy that the extract/transform/load (ETL) process for our last release ran for 37 hours, lights-out, loading 450 million clinical observations from various sources, primarily, a copy of Epic's Clarity store from the KU Hospital.

While i2b2 includes a modern Ajax web front-end, it makes no use of web-style linked data, let alone OWL or realist ontology, for medical terminology alignment, which is a big part of the challenge in making HERON an effective platform for research and for data integration beyond the KUMC enterprise.

While I'm taking a break from heads-down development mode for this conference, and while I'm in Boston, I hope to take time to look more closely at developments such as:

Our Oracle database on SAS drives handles simple user queries in a few seconds, but in some cases it takes a minute or two or fails altogether. After evaluating fusion-io's solid state storage, we ordered 4 fairly large units. We're still working through the operational details of setting it up, but we hope to see considerable performance improvements for both ETL and end-user queries.

I'm also interested to catch up on some W3C stuff: RDF/SQL mapping (no RIF?! darn.), and the Semantic Web Health Care and Life Sciences (HCLS) Interest Group. It should be interesting to compare SPARQL1.1 federated query with the corresponding i2b2 approach: SHRINE.

HERON Update Integrates UKP Clinic data with KUH Hospital Data

This second beta release of the HERON repository includes billing diagnoses and procedures from the UKP clinics (ClinicIdxSource), integrated with data from the hospital (HospitalEpicSource) (#306). The data is current through February 2011 for both sources.

This release also supports querying by whether a patient is in the HICTR? participant registry (#155).

HERON Contents Summary

The repository contains approximately 300 million real observations from the hospital and clinics.

This is down from 470 million due to a defect in the way nursing flowsheet was loaded (#456). We will fix this defect in our next release which will incorporate April data.

HERON, KUH EHR Coverage Varies Over Time

Epic (O2) electronic medical record is the main source of our data. It is the EMR for KUH and increasingly the clinics.

  • Epic went live at KUH on November 12, 2007.
    • It started with the pharmacy, medication administration, and flowsheet modules
    • Physician Clinical Documentation was added in May of 2009.
    • Computerized Provider Order Entry (CPOE) began in November of 2010.
    • Some outpatient clinics owned by the hospital (ex: Jayhawk clinics, outpatient cancer center treatment) began using Epic as earlier but the main UKP clinic rollout of Epic started in Fall 2010. It is planned to complete in 2013.
  • Epic also contains Admission, Discharge Transfer and Patient Registration information from the Hospital Billing System (Seimens SMS). As a result, we have basic demographics on patients going back to the 1980s. But, we lack the rich clinical detail available since 2007.

We receive periodic copies of the Epic Clarity database to use as the foundation of our repository. Our current copy is from September 22, 2010, which, for example, pre-dates rollout of CPOE.

Date-Shifting

Our DeIdentificationStrategy includes shifting the dates of each patient record by up to one year. For example, if a patient was born July 1, 1960 their birth date is shifted back in time between 0 and 365 days, and and all subsequent observations are shifted by the same number of days.

Plans to add Outpatient Billing data to HERON

We plan to incorporate outpatient billing data from GE IDX in our next release. (#306) The IDX data goes back to the 2000 and will include CPT procedure codes and ICD9 diagnoses codes.

De-Identification of Patient Health Information

We have taken several steps to de-identify the patient records so they may be deemed non-human subjects research. This includes:

  • shifting dates as above
  • Removing address identifiers
  • Only providing gross geographic indicators (state and radius from KUMC) (#51)
    • with thanks to geonames for data relating zip codes to latitude/longitude

By default you have obfuscated counting access. This means the exact counts returned by queries are perturbed by a random number of patients. Obfuscation access also means you can't take advantage of analysis tools and timelines. We will work with our clinical partners on providing this greater level of access for our research community. (#314)

Diagnosis Issues

These come from the Epic PAT_ENC_DX table which is populated by the problem list and from tests which are ordered that require a diagnosis. We currently organize the data by ICD9.

  • There are many opinions on this as a source of information. One perspective is that they may be more clinically relevant but lack the completeness of professionally coded diagnoses.
  • In our next release we plan to extract the full ICD9 augmented with SNOMED concepts from the IMO hierarchies stored within Epic. (#273)

We have loaded the top 500 most frequently resulted lab tests

  • These are results that were "finalized" by the lab.
  • These do not include microbiology, blood bank, pathology. Only the general laboratory results.
  • The LOINC coding was not validated by laboratory personnel. We will work towards using the local Epic categorization until the laboratory has time to map their results to national standards.

Medication Issues

  • Medications are the dispensed medications by pharmacy. We believe this data is not accurate and are working towards loading the administered medications from the MAR.
    • For now, you can use medications for exploratory activities but don't take any overall volumes as truth. We believe the medication exposure is underreported.
    • The classification of medications follows the internal Epic categories (based on First Databank).

Nursing Flowsheet Data Issues

  • We have loaded all the nursing flowsheet data with the exception of some dates. (#363)
  • Strings are not visible for privacy preservation but you will at least know that the observation was recorded. (#299)
  • The ontologies for flowsheets is an area of research for us. As a first step we are displaying the internal flowsheet organization within Epic at KUH.
    • Keep in mind that one observation which was documented on one flowsheet may appear on many different flowsheets. The best examples of this are height/weight and vital signs.

Other Data Issues

  • We haven't really loaded procedures and CPTs yet though the ontology hierarchy might lead you to think they are loaded.
  • Searching by age is an interesting topic in i2b2 (#158)
    • For the most part, you are searching based upon the patients current age for events that have occurred in the past.
    • Future releases of i2b2 should allow you to specify search criteria for the age at time of event (more relevant to infant/pediatric queries).

I2B2 Software Platform Issues

For the most part, we are not modifying the i2b2 software. Any defects found may be already shared with the i2b2 development team at Partners.

  • We haven't developed training materials beyond what is provided at the i2b2 home page.
  • We are currently running i2b2 version 1.4. Keep this in mind as you compare current functionality with current and planned i2b2 releases (1.5 and 1.6rc3).
    • We will plan to upgrade to version 1.6 of i2b2 when it is official released by their developers and when we can sequence the upgrade with our production release of HERON. (#165)

Note: we have modified the i2b2 software to display the number of facts at each level of the ontology and in some cases also display the distinct number of patients at each level. (#211)

Getting ahead of the ball with service monitoring

In the earliest phases of development of our HERON clinical research repository, the only users were us developers and a handful of friendly alpha testers, so it was fine to discover problems as we used the system.

But one of the features included in milestone:EpicBetai2b2 is more proactive monitoring (#150), using the popular open source opsview toolset, built on nagios.

Once it was in place for HERON, I showed it to a guy who supports CRIS?, and he figured out how to get nagios working on Windows servers etc.

CRIS is a long-standing production service. Its user community consists of clinical researchers. When CRIS acts up, they don't see it as an interesting technical puzzle to solve; they just see it as the darned computer getting between them and their research goals again.

The database under the CRIS service acted up over the weekend, but this time, instead of a call from a frustrated researcher on Monday morning, the CRIS support team got an automated notice right when the problem started and had it cleaned up before any users noticed.

It's so much nicer to be ahead of the ball in the customer support game.

Concept stats: how many needles are in which parts of our i2b2 haystack?

For the HERON research data repository we're building, we're Using I2B2, which involves navigating a huge vocabulary of medical terminology. We found ourselves wishing that the terms were tagged with some clues about how often they occur in our database. Now that the enhancement ticket is done, we find it really does help.

I2B2 lets researchers identify patient cohorts by querying a database of "observation facts" about patients. All the facts are collected in one table, indexed by concept. Concepts may be demographics (age, gender, etc.), diagnoses, labs, medications, or procedures, and the tool supports hierarchical navigation and text search:

About 14,000 concepts are included with the I2B2 software. As we extract concepts from health records at KU Medical center, some obviously match the i2b2 concepts but many do not. For example, we have not yet determined the relationship between local medication codes and the NDC system. This complicates the already daunting task of navigating the hierarchy of medical terminology. It's quite frustrating to poke around in the dark, running query after query, not knowing which concepts have real data attached to them.

We find that pre-computing the number of facts and the number of associated patients and including these statistics in the hierarchy makes navigation much more efficient:

We can see at a glance that we have no records of dispensing ANTI-OBESITY drugs, at least by that exact categorization.

We can also see that we only have rich data (diagnoses, labs, meds, ...) about roughly 10% of the 2 million patients in our database; this is due to rather recent deployment of an electronic medical record system compared to the long-standing use of computerized billing systems.

How we did it

In the observation_fact table, each of the facts carries a concept_cd; for example:

We then used an expedient hack of updating the concept labels to include the stats:

That code is complicated because it has to

  1. leave out stats regarding fewer than 10 patients (per HIPAA regulations, as part of our DeIdentificationStrategy)
  2. replace any stats that were already there

That's probably enough detail for others using i2b2 to do likewise, but if you're curious, the script that manages the process is source:heron_load/concepts_stats.py and the ticket (#211) tells the whole saga of approaches that don't work and such.
